We want everyone who had battled cancer to be remembered in the

  Memorial Tribute Video-American Cancer Society Relay For Life of Scott County

Families can either mail the form below, a copy of the photograph and donation to:

         *Photographs must be 2x3 to 8x10 in size.

Or email information and photograph to: Relay@ballardsunderfuneral.com

         * JPEG format and in at least a 300 DPI resolution

         * Mail donation to above address, indicating in memory of (who)

We are asking for a $5.00 minimum donation to accompany each photo.

*****Deadline to submit picture is June 20, 2008.

(Mailed in copies of pictures will available for pick up the night of the Relay for Life.)
